Community Services Most adults have life goals they would like to achieve. People with developmental disabilities also have goals for themselves, but reaching them is not that easy. Civitan's Community Services program helps our clients realize their hopes and dreams and fulfill their own vision of a successful life within the community. The end result of skills learned in the Adult Day Program, Community Services has two service components: Independent Living and Supported Employment. Independent Living provides the necessary supports for people with developmental disabilities to live and be a part of a community. Some individuals may need assistance with shopping, accessing community services, assisting with cooking or arranging medical appointments. Others may need more assistance in the form of a live-in attendant. Services are based on individual need and are provided in accordance with the plan of care developed by Civitan Services, the individual, his or her family and a family physician. Supported Employment is the second component of Community Services. The program provides supports for people with developmental disabilities to work in a job of their choice. An extensive profile is developed to determine the skills, abilities and desires of the individual. After generating the profile, job development is conducted to match the needs of the individual with that of an employer. When a job match is made, a Civitan Services Job Coach will train the person at the job site in accordance to the specifications of the employer. This service has been successful both nationally and in Arkansas to assist many people with severe disabilities to gain successful or meaningful employment. Each of these services, Independent Living and Supported Employment, may be utilized separately or together. A person may also choose to be enrolled full-time or part-time in the Civitan Adult Day Service program while in any of the Civitan Community Service programs. Combining these services offers a person a choice of services to meet their specific needs and provides an extra benefit of all services being coordinated by one inclusive organization that functions as a team. Civitan’s customized software program, Streamline IMS, allows each team member immediate access to service documentation, centralized client information, progress reporting and Individual Habilitation Plan analysis. Civitan’s Community Services program provides personalized service and 24 availability to our clients. Just because the work day ends, doesn’t mean that life stops. We are there for our clients any time, day or night as the need dictates.
